Conditions of Works:

1. All safety instructions given by works management must be followed.
2. Personnel listed in this SWAC shall be working in the designated area only, trespassing into other areas is not allowed.
3. Shift duty in-charge (Tel. 21953505/21953508) and responsible Works Supervisor of this work shall be informed prior to commencement of work each day.
4. Contractor shall not use, adjust, switch off or on, or alter the existing plant operation.
5. Within the validity period, the contractor leaves the site for more than 7 working days consecutively or does not commence the site work for more than 10 calendar days after the approved start date. This certificate will be cancelled automatically. The contractor should re-apply for a new certificate.
6. All electrical works shall be done by Registered Electrical Worker of appropriate grade / permitted work.
7. The contractor’s AP should have been obtained Permit-to-Work issued by Authorized Person (Electricity) of DSD before commencement of any electrical works on high voltage equipment. The contractor’s AP shall be on site for supervision throughout the period of electrical works.
8. Separate approval shall be applied for Saturdays, Sundays and Public Holidays.
9. The Contractor shall be fully responsible for isolating/locking the devices, equipment and/or upstream facilities in the presence of DSD’s staff for ensuring safety before commencement of the works, especially working in confined space.
10. The Contractor shall be fully responsible for security/ in safe custody of their property being placed at DSD.
11. Smoking and drinking alcohol are prohibited in all plant areas.
12. All workers entering into / remaining in a confined space, even contractor’s Competent Person has classified no H2S risk within the site area, should properly wear respiration protector with H2S cartridges with max allowable penetration 10 ppm at min air flow rate 30 L/min and operation life span of over 100 min at 100 ppm H2S.
13. All the Risk Assessment & Permit-to-work should be updated immediately once any changes both in working environment and/or method statement of the job.
14. For works at below ground levels, at least two workers must be present to carry out site works.
15. Safety Supervisor (SS) provided under the Contract shall conduct daily site safety inspection during works period.
